Aged Care Comprehensive Assessor

As a Clinical Aged Care Assessor, you'll play a crucial role in identifying the needs of older adults and connecting them with services that help them live independently with the highest quality of life. Daily travel is a core requirement of this role. You will be required to travel throughout your service area every day to meet business and client needs, providing support where it’s needed most.

Responsibilities

Conduct thorough Aged Care Assessments to identify the needs of older adults; Facilitate referrals to essential services that support independent living; Actively promote the retention and maximization of function and capability in daily living; Work closely with families and carers, recognizing them as key stakeholders; Ensure all assessments are conducted in line with Department of Health contractual requirements

Requirements

At least 1 year working with older Australians, including client assessments and/or service delivery; Tertiary education in a health-related discipline together with professional registration with relevant association (e.g. AHPRA); Proficiency in Word, Excel, Outlook, and client management systems; Current Australian driver’s licence, comprehensively insured vehicle, and the ability to travel within your service area; Registered Nurse; Chiropractor; Optometrist; Art Therapist / Music Therapist; Medical Practitioner/Officer; Exercise Physiologist; Counsellor / Psychotherapist / Social Worker / Psychologist; Occupational Therapist; Chinese Medicine Practitioner; Speech Pathologist; Osteopath; Physiotherapist; Dietician; Podiatrist; Paramedic; Radiographer; Dentist; Pharmacist
