Days available - Monday, Tuesday, Saturday & Sunday. Location: Serving Ballarat and surrounding areas. Travel: Must be willing to travel for client care. As a nurse in this role, you’ll work independently while making a meaningful impact on the lives of older Australians.

Responsibilities

Provide health promotion and education to clients and their carers; Promote and deliver Wellness for Independence programs, both in-person and virtually; Monitor and improve health outcomes through Telehealth; Provide direct care services across funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care; Assess, develop, and evaluate nursing care plans collaboratively, focusing on client goals and independence

Requirements

Understanding of community nursing and primary health care principles; At least 2 years of experience in community, acute, hospital or aged care; Experience working independently or as part of a team; Strong organizational skills and attention to detail; Proficiency with electronic devices and client management systems; Current Registration with AHPRA, Police check, working with children check and CPR; Experience providing advanced clinical care, including chronic disease, medication, palliative care, catheter management, stoma care, wound care, and diabetes management; Must be willing to travel for client care
