In-Home Health - Physician (Full Time)- Bridgeport, CT

Signify Health, part of CVS Health, is seeking a Full Time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. The visit lasts about 45 minutes, allows time to connect one-on-one and answer health questions, includes a medication and medical history review, a physical evaluation, and additional diagnostic tests if ordered by the person's health plan. You'll use an iPad and an intuitive clinical workflow for each evaluation.

Responsibilities

Provide In-Home Health Evaluations to health plan members in their homes; Connect one-on-one with members to answer health questions; Conduct medication and medical history review; Perform physical evaluation; Administer additional diagnostic tests (diabetic eye exams, spirometry, etc.) as ordered; Use iPad and intuitive clinical workflow for each evaluation; Travel locally and within licensed states as needed; Conduct virtual visits as needed; Provide Diagnostic & Preventive Services products (spirometry, DEE, bone density screening, etc.)

Requirements

MD or DO; Active, unrestricted license(s) in coverage area(s); Able to cover the following primary locations: Bridgeport and surrounding area; Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
