In-Home Physician (Per Diem) - Scotts Bluff, NE

Signify Health, part of CVS Health, is seeking a Per Diem Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

Responsibilities

Perform 45-minute in-home health evaluations including medication and medical history review and physical evaluation; Conduct additional diagnostic tests if ordered by health plan (e.g., diabetic eye exams, spirometry); Use an iPad and intuitive clinical workflow for each evaluation; Provide Signify Health services including Diagnostic & Preventive Services products (spirometry, DEE, bone density screening); Conduct virtual visits as needed; Travel locally and within licensed states as needed; Build trusted relationships with health plan members in their homes

Requirements

MD or DO; Active unrestricted license(s) in coverage area(s); Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants; Flexibility to travel locally and within licensed states as needed
