Medical Social Worker- Care Coordination - PRN Days

Utilizes professional social work skills in collaborating with other members of the medical team to meet the needs of patients and works with patients (either individually or in groups) to prevent or ameliorate related social problems.

Responsibilities

Provide social work services to patients/families for an assigned area; Coordinate all discharge planning activities for complex patients as assigned; Help resolve psychosocial problems which interfere with treatment or rehabilitation; Serve as a consultant to the medical staff on the effect of socio-economic factors on patients' health problems; Extend and integrate the hospital with other health, educational, and welfare resources in the community; Make appropriate community/home visits to ascertain the environmental background of clients; Provide case management services and document each activity in the case record; Coordinate groups of various kinds: educational, support, etc.; Contribute to Administrative planning; Other duties as assigned

Requirements

Master's of Social Work; Some hospital experience
