RN - Integrated Nurse Case Manager - PRN

The Integrated Nursing Case Manager, under the direction of the Director/Manager of Case Management, provides care/service safely and efficiently for a full range of services to patients of all ages and their families. Primary role is to collaborate, communicate and facilitate coordination of services during hospitalization and post-hospitalization as established by the healthcare team. Integrated Nursing Case Manager concurrently facilitates proactive patient care services to ensure that care is appropriate, timely, and cost-effective and achieves the desired outcome. The Integrated Nursing Case Manager coordinates the care and service of selected patient populations across the continuum of illness; promotes effective utilization and monitoring of health care resources; and guides all disciplines toward positive quality outcomes.

Responsibilities

Provides care/service safely and efficiently for patients of all ages and their families; Collaborates, communicates and facilitates coordination of services during hospitalization and post-hospitalization; Facilitates proactive patient care services to ensure care is appropriate, timely, and cost-effective; Coordinates care and service of selected patient populations across the continuum of illness; Promotes effective utilization and monitoring of health care resources; Advocates on behalf of patients and caregivers for identification and access to services; Conducts admission and continued stay reviews to evaluate medical necessity and appropriateness of treatment plan; Facilitates external payer certification for hospital and discharge needs; Facilitates discharge planning, resource referral and patient education; Coordinates and facilitates communication among all team members, including community providers; Monitors individual patient outcomes and intervenes as necessary; Participates in performance improvement activities; Acts as a preceptor for new team members; Assists in training of new team members

Requirements

High School Graduate; Associate Degree Nursing; Licensed Registered Nurse (LRN) - Single State - State Board of Nursing Registered Nurse in Kansas; Current active multistate state Registered Nurse (RN) license in good standing in state in which you live OR active single state RN license in good standing in the state you will be working with proof of application of a multistate RN license in the state in which you live; Basic Cardiac Life Support (BLS or BCLS) - American Heart Association (AHA); Ability to proficiently read and document in electronic medical record; Basic typing and word processing ability; High degree of cognitive skill - strong attention to detail

Nice to have

Bachelor Degree Nursing; 2 or more years clinical experience; 2 or more years clinical experience in a health care setting
