Social Services Associate, Discharge Planning

Solaris HealthCare Coconut Creek is now hiring. We are looking for caring individuals to provide outstanding care to our residents each and every day. The primary purpose of this position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of our Social Services Department to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Responsibilities

Assist in planning, developing, organizing, implementing, evaluating, and directing the social service programs; Participate in discharge planning, resident assessments, and development and implementation of social service care plans; Interview residents/families to obtain social history information; Provide information to residents/families regarding Medicare/Medicaid and other financial assistance programs; Refer residents/families to appropriate social service agencies; Coordinate social service activities with other departments; Evaluate social and family information and assist in determining plans for social service interventions; Ensure social service progress notes are current, informative, and descriptive of services provided; Maintain a quality working relationship with medical professionals and other health related facilities and organizations

Requirements

MSW or BSW; Florida AHCA Clearinghouse background screening

Nice to have

Bachelor's degree in social work or human services field; One year supervised social work experience in a health care setting; Discharge planning experience
