Street Outreach Manager

Street Outreach Manager (Program Leadership – Field-Based). The Street Outreach Manager leads field-based outreach operations with a dual focus on generating high-quality member referrals into CalAIM Enhanced Care Management (ECM), Community Supports (CS), and Behavioral Health (BH), while building durable, strategic relationships with community-based organizations (CBOs). This role is accountable for outreach performance, referral pipeline development, and the establishment of trusted partnerships that drive sustained referral flow, coordinated care, and improved member outcomes.

Responsibilities

Lead and oversee outreach operations within hiring county; Spend time in the field supporting outreach staff, partners, and referral development; Manage outreach performance, referral pipelines, and partnership growth; Collaborate with ECM, CS, and Behavioral Health teams to ensure alignment and handoffs; Track performance metrics and adjust strategies in real-time; Build and maintain strong community presence across the county; Lead outreach strategies to identify, engage, and refer ECM-, CS-, and BH-eligible members; Establish and sustain relationships with shelters, housing providers, street medicine teams, behavioral health providers, and community organizations; Supervise and coach outreach staff; Oversee daily outreach activities; Maintain strong knowledge of ECM eligibility pathways and Community Supports services; Ensure accurate documentation of outreach, referrals, and partnerships.

Requirements

5+ years in community outreach, healthcare, social services, or related field; Leadership experience supervising field-based or outreach teams; Strong understanding of Medi-Cal, CalAIM, ECM, Community Supports, and Behavioral Health programs; Proven experience building and maintaining community partnerships; Strong communication, leadership, and relationship management skills; Ability to manage multiple priorities and partnerships effectively; Strong technical proficiency, including CRM systems, reporting tools, and documentation platforms; Ability to analyze data and translate insights into actionable strategies; Ability to travel extensively within hiring county; Ability to work occasional evenings or weekends for community events; Ability to work in a hybrid, field-based leadership role; Reliable personal vehicle for daily work use; Successful completion of background check (including MVR); Must be able to travel up to 60-70% within the county to conduct in person visits; Must successfully complete a Testlify skills assessment; Must have a reliable working laptop for the first 21 days of employment (personal equipment stipend) until company issues laptop is received; Must have effective Time Management skills; Must have internet speed of - 300+ mbps download and 25+mbps upload; Must be proficient in technology, including documentation systems, case management platforms, and communication tools; Must be willing to work occasional evenings and weekends as needed.
