Aged Care Comprehensive Assessor

Join our growing team of assessors working out in the Fremantle Region of WA and from your own home. As a Clinical Aged Care Assessor, you will play a crucial role in providing older adults access to clinical based assessments in their home. In doing so, you are connecting clients with services that help them live at home with the highest quality of life for longer. Daily travel is a core requirement of this role. You will be required to travel throughout your service area every day to meet business and client needs, providing support where it’s needed most.

Responsibilities

Travel throughout service area daily to meet business and client needs; Conduct thorough Aged Care Assessments to identify needs of older adults; Facilitate referrals to essential services supporting independent living; Promote retention and maximization of function and capability in daily living; Work closely with families and carers as key stakeholders; Ensure assessments are conducted in line with Department of Health contractual requirements

Requirements

At least 1 year in aged care, including client assessments and/or service delivery; Tertiary education in a health-related discipline (e.g. nursing, occupational therapy, physiotherapy, social work); Current unrestricted General Registration with AHPRA or relevant professional association; Proficiency in Word, Excel, Outlook and client management systems; Current Australian driver licence, comprehensively insured vehicle and the ability to travel within your service area; Must be a Permanent Resident or Citizen due to contractual requirements; Successful completion of mandatory pre-employment checks
