Care Coordinator - Community Youth Services - FT - Days - CYS

Provides support to community youth and their families including informal counseling, care coordination, and overseeing the daily operations of the program's services.

Responsibilities

Develops and maintains daily activity schedules; Transports clients and families to appointments and/or community resources, when needed; Per grant requirements, completes surveys and/or assessments with clients and families and reports outcome data to program's Team Leader; Provides case management, care coordination, and informal counseling for clients and families enrolled in program; Evaluates and assesses the needs of the clients and families

Requirements

Bachelors; Automobile Insurance; BLS American Heart E-Card; BLS American Heart RQI E-Card; BLS Cert American Heart_non ecard; BLS Certification Grace; BLS Cert Red Cross; Driver License Num; Valid Florida driver's license; Cleared by Legal to be added to Memorial Healthcare System's insurance; Personal vehicle available during work hours; Extensive travel throughout Broward County; No experience needed; Requires critical thinking skills, effective communication (written and verbal) skills, decisive judgment and the ability to work with minimal supervision; Must be able to work in a stressful environment and take appropriate action; Must be able to handle a heavy case load and multi-task; Must be able to work a flexible schedule
