Experienced Community Physiotherapist - Aged Care

Help older Australians stay independent, active and confident in their own homes. Right at Home Gold Coast is seeking a passionate and motivated Physiotherapist to join its growing community aged care team. The role provides person-centred, evidence-based physiotherapy to older clients in their homes and community settings, with a focus on helping clients remain safe, mobile and independent.

Responsibilities

Completing comprehensive physiotherapy and functional assessments; Developing individualised treatment and rehabilitation plans; Supporting mobility, strength, balance and functional independence; Falls risk assessment, prevention and management; Prescribing appropriate exercise and home-based rehabilitation programs; Assessing mobility and recommending appropriate aids or equipment; Supporting clients living with chronic health conditions and age-related changes; Educating clients, families and Care Partners about safe mobility and exercise; Monitoring client progress and adjusting interventions as required; Working collaboratively with Nurses, Care Partners, GPs, allied health professionals and families; Maintaining accurate clinical documentation and care plans; Identifying changes in client health or function and escalating concerns appropriately

Requirements

Bachelor's or Master's degree in Physiotherapy; Current AHPRA registration; Current Australian driver's licence; Strong clinical reasoning and assessment skills; Excellent communication and interpersonal skills; The ability to work independently within clients' homes; A commitment to person-centred and evidence-based care; An interest in aged care, rehabilitation, healthy ageing and maintaining independence
