In-Home Health - Nurse Practitioner or Physician Assistant (Per Diem) - Pullman, WA

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person’s health plan, additional diagnostic tests. As an in-home clinician, the primary workplace is the private residence of health plan members.

Responsibilities

Provide In-Home Health Evaluations; Connect one-on-one with health plan members and answer health questions; Conduct medication and medical history review; Perform physical evaluation; Perform additional diagnostic tests if ordered by the health plan (e.g., diabetic eye exams, spirometry); Travel locally and within licensed states as needed; Conduct virtual visits as needed; Provide Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)

Requirements

Active, unrestricted license(s) in coverage area(s) PULLMAN ASOTIN; Board certification; Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
