In-Home NP Clinician

Signify Health, part of CVS Health®, is hiring part-time board-certified nurse practitioners to conduct In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members.

Responsibilities

Conduct pre-scheduled in-home visits with health plan members, traveling primarily locally within a designated metro area; Provide at least 12 hours of availability weekly, consistently providing 50+ capacity units per month; Execute a strong visit completion rate, pacing 35+ completed visits per month; Conduct Video Visits as needed; Maintain training and complete all diagnostic and preventive service (DPS) products as directed; Maintain required supplies and associated diagnostic equipment related to DPS offerings; Accommodate field observations and/or ridealong experiences as requested

Requirements

Board-certified nurse practitioner (NP); Active state license in coverage area; Maintain board certification; Provide at least 12 hours of availability weekly and 50+ capacity units per month; Execute a strong visit completion rate, pacing 35+ completed visits per month; Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s); Be accountable to Signify Health operational, clinical, and member experience KPIs and metrics; Conduct Video Visits as needed and maintain training, affiliation, and compliance with Video Visit requirements; Maintain training and complete all diagnostic and preventive service (DPS) products as directed; Maintain required supplies and associated diagnostic equipment related to DPS offerings; Be flexible to travel locally and within licensed states up to 25%; Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies; Accommodate field observations and/or ridealong experiences as requested

Nice to have

FNP, AGNP certification preferred; Multi-state licenses preferred; Experience with Signify Health In-Home Health Evaluations, in-home visits, or similar format preferred
