Physical Therapist

Banner Baywood Medical Center, a 342-bed hospital in Mesa, Arizona, is seeking a Physical Therapist for the Banner Baywood Acute Care, Rehab team. The position is responsible for the evaluation, planning, administration and supervision of Physical Therapy treatment in accordance with physician referral, state and national organizational standards, regulatory agencies, and established departmental policies and procedures. The shift is Monday, Tuesday, Thursday and Friday with one weekend per month requirement (first or third weekend). Work shift: Day. Department: Thrpy-Acute-Hosp. Job category: Clinical Care.

Responsibilities

Evaluates patient functional abilities and limitations, establishes plan of care and treatment goals, and initiates discharge planning; Implements individualized treatment plans and assesses ongoing effectiveness, modifying the plan of care as indicated; Involves patient and family/caregiver in treatment and decision making; Provides ongoing education to physicians, nurses, patients, family/caregivers and the interdisciplinary team; Documents according to professional practice guidelines and regulatory agency standards within department time frames; Communicates evaluation results and patient progress to physicians, nurses, patients, family/caregivers and the interdisciplinary team; Demonstrates appropriate supervision and delegation of PTA, ATC, Therapy Technician and student personnel; Participates in department meetings, process improvement and training of new personnel; Meets departmental productivity standards; Works in all areas of the hospital including Med-Surg, Ortho, Neurology, ICU/CCU, CVICU, PCU, ED and Observation

Requirements

Must have graduated from an APTA accredited school of Physical Therapy, or equivalent; Must possess a current Physical Therapy license within the State of practice; Internationally trained candidates have six months from the time of hire to obtain a current license (per California regulations); BLS certification required; Must have the ability to become proficient with computer software applications commonly used in an automated patient care environment; Employees working for Banner Home Care/Hospice or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain the card for the duration of their employment
