Physical Therapist- Outpatient Rehab

Responsibilities

Interprets physician referrals and completes musculoskeletal, neuromuscular, cardiopulmonary, and/or integumentary evaluations; Establishes a plan of care and implements individual treatment based on evaluation results and functional goals; Monitors and evaluates outcomes and adjusts the individualized treatment plan; Plans for and completes patient discharge including written home programs, patient/family teaching, referral for community services, transition planning and ordering equipment; Completes all required documentation including patient evaluations, progress notes, team conference reports, and discharge summaries; Records patient care and progress, completes daily statistics, billing, and scheduling; Applies insurance guidelines regarding authorization and number of visits; Maintains site/department productivity standards and adheres to billing and documentation policies; Cleans treatment area, puts away equipment, and keeps area neat and orderly; Performs manual techniques, patient positioning, transferring and transporting duties; Demonstrates skill in use of modalities, exercise equipment and other devices; Promotes patient safety and follows safety and infection control measures; Participates in performance improvement activities; Manages assigned caseload and assists with coverage; Pursues professional growth through continuing education, department in-services, and professional literature; Orients new staff and participates in evaluation of co-workers; May supervise physical therapy assistants, extenders, and students

Requirements

Doctorate degree in Physical Therapy from an accredited school or educational requirements commensurate with the date of graduation; Maintains continuing education requirements as mandated by applicable state professional standards; Licensed Physical Therapist in the State upon which you work; Basic Life Support (BLS) for Healthcare Providers by American Heart Association (obtained within 90 days unless department leader has determined is not required); 1-2 years of experience in Pelvic Health or Lymphedema; Intermediate computer skills; Excellent communication, interpersonal, and organization skills; Demonstrated organization, decision-making, and problem-solving skills; Successfully completes all competency assessments/requirements pertaining to provision of patient care; Maintains annual educational requirements for hospital-based certifications; Ability to demonstrate knowledge and skills necessary to provide age-appropriate care; Must be able to sit, stand, walk, lift, and squat throughout the workday; Ability to lift up to 50 lbs from floor to waist, up to 10 lbs from waist to overhead, and lift/carry up to 40 lbs at waist height; Ability to push/pull with 30 lbs of force and perform a sliding transfer of 150 lbs with a second person; Repetitive use of hands including simple grasping and fine manipulation; Functional use of senses for effective communication; Ability to wear protective clothing as needed; Ability to travel; Ability to operate all equipment necessary to perform the job
