Social Services Associate, Discharge Planning

Solaris HealthCare Coconut Creek is now hiring. We are looking for caring individuals to provide outstanding care to our residents each and every day. The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of our Solaris HealthCare's Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Director of Social Services and/or Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Responsibilities

Assist in planning, developing, organizing, implementing, evaluating, and directing social service programs; Participate in discharge planning, resident assessments, and development/implementation of social service care plans; Interview residents/families to obtain social history information; Refer residents/families to appropriate social service agencies; Provide consultation to staff and community agencies; Maintain quality working relationship with medical professionals; Assist with Medicare/Medicaid and other financial assistance programs; Ensure social service progress notes are current and descriptive

Requirements

MSW or BSW; Florida AHCA Clearinghouse background screening

Nice to have

Bachelor's degree in social work or human services field; One (1) year supervised social work experience in a health care setting; Discharge planning experience preferred
