Social Services Associate, Discharge Planning

The primary purpose of your job position is to assist in planning, organizing, implementing, evaluating, and directing the overall operation of our Solaris HealthCare’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Director of Social Services and/or Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Responsibilities

Assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department; ensure that the medically related emotional and social needs of the resident are met/maintained; participate in discharge planning, resident assessments, and development and implementation of social service care plans; interview residents/families to obtain social history information; refer residents/families to appropriate social service agencies; provide information to residents/families regarding Medicare/Medicaid and other financial assistance programs; coordinate social service activities with other departments; maintain a quality working relationship with medical professionals and other health related facilities; ensure social service progress notes are current, informative and descriptive

Requirements

MSW or BSW; Florida AHCA Clearinghouse background screening

Nice to have

Bachelor’s degree in social work or a human services field including sociology, gerontology, special education, rehabilitation counseling, and psychology; one year supervised social work experience in a health care setting working directly with individuals; discharge planning experience preferred
