Social Worker Per Diem - Inpatient Outpatient Hospice

PRN Social Worker Position involving both Inpatient and Outpatient Hospice. Must have flexibility to cover both weekend and some weekday coverage. Master's Degree and 2 years of field work required. Interviews patients and relatives to obtain social history relevant to patient's medical problems and assess bereavement risk relative to anticipatory grief and family members needs post death. Assists patients with environmental difficulties that interfere with obtaining maximum benefits from medical care. Serves as liaison between medical and nursing staffs, patients, relatives, and appropriate outside agencies. Interprets and assists in resolving social problems that relate to medical condition and/or hospitalization.

Responsibilities

Interview patients and relatives to obtain social history relevant to patient's medical problems and assess bereavement risk; Assist patients with environmental difficulties interfering with medical care; Serve as liaison between medical/nursing staff, patients, relatives, and appropriate outside agencies; Interpret and assist in resolving social problems related to medical condition or hospitalization; Develop and implement patient's care plan; Provide individual and group counseling and bereavement follow-up; Act as navigator and entry point for marginalized community members to essential life enhancing resources; Maintain current, accurate documentation in the client's medical record

Requirements

Master's degree in Social Work or a related field; For Hospice, Master's degree in Social Work; Two years of social work experience; Flexibility to cover weekend and some weekday coverage; HeartSaver First Aid/CPR or Basic Life Support (BLS) certification; Valid AZ fingerprint clearance card may be required per department; Knowledge of interviewing techniques; Skill in assessing socio-economic factors; Ability to effectively present information

Nice to have

Licensure as LCSW or LMSW preferred; Two years of social work experience in an acute care setting preferred; Behavioral health, substance abuse/treatment care setting experience strongly preferred
