Care Partner

Right at Home Southwest Victoria is growing its Clinical Care Team and is looking for someone who excels in communicating and building relationships to join as a Care Partner. The role, open to Enrolled Nurse, Occupational Therapist, Social Worker and PCW candidates, involves becoming the primary point of contact for an allocated portfolio of clients and delivering person-centred care, with regular weekday hours.

Responsibilities

Managing an allocated portfolio of clients; Developing and monitoring care plans; Completing routine client reviews and follow-up; Building strong relationships with clients and their representatives; Identifying changes in client needs and coordinating appropriate responses; Working collaboratively with the Care Manager and Clinical Care Team Leader; Supporting high-quality, person-centred care across the organisation

Requirements

Qualification in either Enrolled Nurse/Endorsed Enrolled Nurse (registered with AHPRA), Social Worker, Occupational Therapy or Cert III/IV in Individual Support (with previous experience as a Care Partner); Current Driver Licence; National Police Check; Current First Aid and CPR
