Responsibilities

Provide skilled treatment under PT supervision, including gait training, therapeutic exercise, balance and fall prevention, and neuromuscular re-education; Carry out individualized plans of care to improve mobility, function, and safety in the home; Educate patients and caregivers on home exercise programs, assistive devices, and home safety; Communicate patient progress and changes to your supervising PT and the care team; Complete documentation using AI-assisted tools

Requirements

Current California PTA license (required); Valid California driver's license with a clean record (required); Home health experience preferred; Strong outpatient, SNF, or hospital PTAs are encouraged to apply; Ability to travel regularly within San Francisco County
