In-Home Nurse Practitioner or Physician Assistant Part Time - Lynchburg

Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. The clinician plays a critical role in building trusted relationships to make people healthier, helping bring flexible, comprehensive and personalized health evaluations right to people's front doors.

Responsibilities

Provide In-Home Health Evaluations; Build trusted relationships to make people healthier; Conduct medication and medical history review, physical evaluation, and additional diagnostic tests if ordered; Use an iPad and intuitive clinical workflow for each evaluation; Travel locally and within licensed states as needed; Conduct virtual visits as needed; Provide Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)

Requirements

Active, unrestricted license(s) in coverage area(s); Board certification; Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
