In-Home Nurse Practitioner or Physician Assistant Part Time - Rappahannock

Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

Responsibilities

Provide In-Home Health Evaluations; Engage with people in the comfort of their own homes; Complete 45-minute visits including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered; Use an iPad and intuitive clinical workflow; Be flexible to travel locally and within licensed states; Conduct virtual visits as needed; Provide all Signify Health services including Diagnostic & Preventive Services products such as spirometry, DEE, and bone density screening

Requirements

Active, unrestricted license(s) in coverage area(s); Board certification; Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
