Intake Specialist - Regional Center

Care Partners At Home provides compassionate, reliable, and personalized home care services. The Intake Specialist - Benefits role is responsible for serving as the primary point of contact for new clients and families, conducting intake assessments, coordinating personalized care plans, maintaining compliant documentation, and building relationships with clients, families, and Regional Center and/or Cal AIM representatives. Work schedule: Monday - Friday, 8:00 am-5 pm.

Responsibilities

Serve as the primary point of contact for new clients and their families; Conduct thorough intake assessments to identify client needs and preferences; Collaborate with Regional Centers, Cal AIM, caregivers, and internal teams to design and implement personalized care plans; Maintain accurate records and documentation in the CRM system; Ensure all intake processes comply with state regulations and Regional Center and/or Cal AIM requirements; Develop and maintain positive relationships with clients, families, and Regional Center and/or Cal AIM representatives; Act as a trusted resource and advocate for families navigating the services system; Work closely with the Management and Operations teams to streamline processes and ensure high-quality service delivery; Provide feedback and recommendations to enhance the client intake experience

Requirements

Experience in intake coordination, case management, or a related role; Exceptional interpersonal and communication skills, with a compassionate and professional demeanor; Proficiency in computer systems, including Microsoft Office, CRM platforms, and internet-based applications; Ability to pass a federal background screen; Valid driver's license and proof of current auto insurance
