Responsibilities

Perform comprehensive medicines reconciliation and clinical screening; Provide ward-based clinical pharmacy services within mental health units; Support safe prescribing and medicines optimisation for mental health patients; Work closely with multidisciplinary teams to deliver holistic care; Provide patient counselling and medication advice; Maintain accurate clinical records

Requirements

GPhC registration; Previous hospital ward experience; Strong clinical knowledge and communication skills; Ability to work independently and within a multidisciplinary team; Commitment to high-quality patient care
