Personal Care Worker

The Personal Care Worker assists in the provision of holistic client focused and consumer directed care in accordance with an established care plan. The Personal Care Worker provides for the personal needs and comfort of clients in their own home.

Responsibilities

Assists in the provision of holistic, client-focused and consumer-directed care in accordance with an established care plan; Provides personal care activities contained in the written client care plan to assist with activities of daily living, including personal hygiene, toileting, nutrition, and mobility; Actively participates in identifying, implementing and evaluating client care needs in their home in conjunction with the Registered Nurse; Performs light housekeeping tasks that facilitate the client's self-care at home; Documents services provided using the Visual Care app and the client care record; Refers all questions, reports of client concerns or changes in condition immediately to the Registered Nurse On-Call

Requirements

Certificate III in Aged Care
