Pharmacy Tech - Med Rec - Per Diem

This position is accountable for participating in medication reconciliation for patients who have been identified as high risk for medication discrepancies. This role involves obtaining comprehensice medication histories interviews with patients caregivers, as well as communication with outpatient pharmacies and other sources. The technician accurately documents and updates the medication list in the electronic health record. The technician will communicate completion of medication reconciliation and identified discrepancies to the patient's care team (physician, nurse and pharmacist). The technician will also be responsible for participating in Quality Assurance/Quality Improvement projects related to medication reconciliation.

Responsibilities

Participating in medication reconciliation for patients who have been identified as high risk for medication discrepancies; Obtaining comprehensive medication histories through interviews with patients and caregivers; Communicating with outpatient pharmacies and other sources; Accurately documenting and updating the medication list in the electronic health record; Communicating completion of medication reconciliation and identified discrepancies to the patient's care team (physician, nurse and pharmacist); Participating in Quality Assurance/Quality Improvement projects related to medication reconciliation.

Requirements

High School Diploma/G.E.D. (required); CPHT - Certified Pharmacy Tech - PTCB - Pharmacy Technician Certification Board (required); TCH-Pharmacy Technician License - PTCB - Pharmacy Technician Certification Board (required); Minimum three (3) years Hospital pharmacy, medical clinic, or retail pharmacy experience. (required); Computer Skills Proficiency with the Microsoft Office Suite.; Demonstrated knowledge of outpatient and retail drugs (prescription and non-prescription) and usual pediatric doses.
