Primary Care Staff Nurse Per Diem

This role provides clinically based, risk reducing care and coordination that facilitates the delivery of cost-effective quality healthcare along the continuum. It directly interfaces with physicians, care managers, health care teams, patients and their caregivers to achieve desired patient outcomes. Manages individualized, outcome-oriented nursing care through the use of the nursing process for pediatric and adult patients. Provides care, treatment and services through the successful coordination and completion of a series of processes that include assessment, planning care, providing care, coordinating cross continuum care and assessing outcomes of interventions in accordance with system, departmental and ambulatory policies and procedures.

Responsibilities

Provides clinically based, risk reducing care and coordination; Directly interfaces with physicians, care managers, health care teams, patients and caregivers; Manages individualized, outcome-oriented nursing care through the nursing process for pediatric and adult patients; Administers and documents medications; Completes medication reconciliation; Develops, discusses, and communicates a prioritized plan of care; Initiates patient outreach and identifies patient/family needs; Facilitates care across the continuum; Fosters a culture of safety; Utilizes technological solutions including Outlook, Epic, HealthStream and Workday; Participates in quality improvement efforts, research and evaluation processes; Conveys respect for patient and family values and preferences

Requirements

Graduate of an accredited School of Nursing; RN - Registered Nurse; BLS - Basic Life Support; Demonstrated proficiency in ambulatory care nursing, knowledge, and skills; Excellent verbal and written communication skills; Demonstrates flexibility via an ability to adapt to changing priorities and regulations; Basic computer skills; Demonstrated skills in communication (verbal and written), interdisciplinary collaboration, creative problem solving, and critical thinking; ACLS (AHA) Advanced Cardiac Life Support Certification - American Heart Association (AHA); INSTRUCTOR- Advanced Cardiac Life Support (ACLS) - American Heart Association (AHA); INSTRUCTOR- Basic Life Support (BLS) - American Heart Association (AHA); RN-Registered Nurse - Board of Registration in Nursing (Massachusetts)

Nice to have

BSN preferred; Previous Ambulatory Care RN experience preferred; Knowledge of health system and community resources preferred
