Registered Nurse (RN) - Home Health

Full-time salaried Registered Nurse position in the Home Health Nursing department, Cleveland region, with weekend days schedule and 40 hours per week at Advocate Health, Shelby, NC.

Responsibilities

Responsible for the daily management of an assigned patient caseload, ensuring timely care and documentation per the plan of care and MD orders; Collects data from patient assessments in the home setting, telephonic contacts, and/or EMR information; Analyzes data to determine issues and resources, and identifies patients at high risk for re-hospitalization, falls, or high resource utilization; Prioritizes multi-issue situations based on patient health and welfare, patient/staff safety, and timely services; Identifies barriers to resolution (e.g., language, literacy, psychological) and follows up with appropriate resources or leadership; Develops and provides oversight of a plan of care communicated to all parties; Implements the plan and directs care coordination of a multidisciplinary team, including supervision of LPNs and/or HHAs; Performs ordered treatments, therapies, and/or procedures in accordance with the State Nurse Practice Act, Home Health Conditions of Participation, JCAHO, and agency policy; Evaluates progress toward patient-centered goals

Requirements

Graduate from an accredited School of Nursing; BLS required per policy guidelines; Current RN license or temporary license as a Registered Nurse Petitioner in the state in which you work and reside, or meet the licensure requirements of the applicable National License Compact or non-compact state rules; Must speak English in good, understandable terms; Ability to lift and support 35 pounds; Walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling; Intact sense of sight, hearing, smell, and touch; Finger dexterity; Critical thinking and ability to concentrate; Ability to respond quickly to changes in patient conditions; Physical Abilities Testing required; Additional education, training, certifications, or experience may be required by the nurse leader
