SNF Full-Time NP/PA

Atrium Health invites Physician Assistants and Nurse Practitioners to join the Adult Medical Specialty team within the Post Acute Network — Skilled Nursing Facility (SNF) program in Charlotte, NC. Position highlights include Monday–Friday 8:00 AM–5:00 PM, shared call rotation, in-person and virtual visits supported by Medical Assistant, managing complex patients with ongoing follow-up throughout stay, collaborating with SNF team to facilitate care coordination, and implementing discharge planning to improve patient outcomes.

Requirements

Physician Assistant or Nurse Practitioner; Passion for working in a SNF/Post-Acute Care Environment
