Aged Care Comprehensive Assessor

Join our growing team of assessors working out in the Mount Barker Region of SA and from your own home. As a Clinical Aged Care Assessor, you will play a crucial role in providing older adults access to clinical based assessments in their home. Daily travel is a core requirement of this role.

Responsibilities

Conduct thorough Aged Care Assessments to identify the needs of older adults; Facilitate referrals to essential services that support independent living; Actively promote retention and maximization of function and capability in daily living; Work closely with families and carers, recognizing them as key stakeholders; Ensure all assessments are conducted in line with Department of Health contractual requirements; Travel throughout the service area daily to meet business and client needs

Requirements

At least 1 year in aged care, including client assessments and/or service delivery; Tertiary education in a health-related discipline (e.g. nursing, occupational therapy, physiotherapy, social work); Current unrestricted General Registration with AHPRA or relevant professional association; Proficiency in Word, Excel, Outlook and client management systems; Current Australian driver licence, comprehensively insured vehicle and the ability to travel within your service area; Must be a Permanent Resident or Citizen due to contractual requirements; Successful completion of mandatory pre-employment checks
