Care Planner

As a Care Planner within our Short Term Care at Home Service, you’ll play a vital role in ensuring our service users receive safe, effective, and person-centred care.

Responsibilities

Complete and review care plans tailored to individual needs; Carry out risk assessments to ensure safe delivery of care; Work closely with care teams to maintain high standards; Support service users transitioning through short-term care; Ensure all documentation is accurate and up to date; Contribute to delivering high-quality, outcome-focused care

Requirements

Full UK driving licence; access to own vehicle; Organised and detail-oriented; Compassionate and person-centred in approach; Confident working independently and as part of a team; Committed to delivering high-quality care
