Clinical Coordinator, Registered Nurse, Home Care Team 4, Full Time, Day

To oversee and coordinate the overall clinical operations, functions, and activities of the Home Care Team to include, but not limited to: ensuring efficient processing of referrals, evaluating services, handling scheduling to include weekend scheduling, and supervising assigned staff.

Requirements

Graduate from a NLN accredited School of Nursing; At least two (2) years experience as a RN in Home Care or community health; Current State of New Jersey license (or legal permission to work from the New Jersey Board of Nursing); CPR (BCLS) Certification; Demonstrates effective interaction and communication (oral, writing, presenting) skills; Effective organization skills and ability to perform work accurately and pay attention to details; Ability to function competently in stressful situations and a changing work environment; Ability to work cooperatively within the health-system, with patients and family members, and with multidisciplinary team members; Ability to utilize effective time management and respond quickly to emergency situations; Ability to use critical thinking; Current and valid driver's license, registration, inspection sticker and appropriate level of insurance coverage, including collision; Adherence to HR Policy 108 including an acceptable driving record for the duration of employment as long as driving remains a job responsibility

Nice to have

BSN or Associate RN from a NLN accredited school; Supervisory experience
