Clinical Pharmacist

Responsibilities

Engage with patients through outbound calls to provide personalized medication support, with an expectation of approximately 100 calls per day; Perform comprehensive medication reviews; Deliver medication therapy management services; Accurately and efficiently transcribe patient-specific prescription and diagnosis information, along with recommendations, into a web-based application to generate personalized patient documents; Participate in continuous learning and training sessions to enhance skills and stay current with best practices

Requirements

Current and valid Pharmacist License in good standing; At least 4 years of experience as a clinical professional specializing in medication therapy management; Familiarity with key quality measures related to medication use and patient safety; Strong understanding of pharmaceutical and medical vocabulary; Demonstrated clinical experience in medication review processes; Fluency in spoken and written English; Excellent verbal and written communication skills; Proficiency with technology and various software applications; Self-directed and highly organized; Ability to maintain strict confidentiality and discretion; Exceptional attention to detail; Proactive approach and a strong sense of urgency; Must be authorized to work in the United States and not require sponsorship; Must be able to work on a W-2 basis, as C2C or 1099 is not permitted
