Clinical Supervisor: Physical Therapy Home Health

The Clinical Supervisor - Therapy is responsible for ensuring that patient care is coordinated and managed appropriately, serves as the first point of contact for field clinicians, and supervises clinical personnel. This is a hybrid role with 2-3 days in the San Jose office and 3 days in the field (average 15+ patient visits each week, including supervisory visits of field clinicians). Coverage area: San Jose and South Bay. All opportunities at 21HHS require being in the field, visiting patients in their homes.

Responsibilities

Responsible for in-the-field oversight of clinicians who provide care and treatment to patients in their place of residence; Ensures patient care is coordinated and managed appropriately; Serves as the first point of contact for field clinicians to assist in patient care escalations; Handles incoming calls timely and follows up as necessary to meet internal and external customer needs; Coordinates training and educational programs for nurses in the field to improve clinical outcomes and competencies; Assesses staff learning needs and formulates plans and goals with employees; Coordinates at least bi-weekly Case Conferences with the clinical team to discuss patient progress and revise care plans; Reviews and evaluates each case, including services provided, conferences, and record review; Reviews patient clinical diagnosis, medications, procedures and clinical course; Assists clinicians in establishing therapeutic goals, setting priorities, and developing plans of care

Requirements

Two years management or supervisory experience in a rehabilitation therapy department or program, preferably home care; Possesses a degree from a baccalaureate or masters program in applicable discipline approved by an accredited organization; Current CA Physical Therapist license; Current Basic Life Support (BLS); Valid CA Drivers License with acceptable driving record; All license and certifications must be current at time of hire and sustained throughout employment
