Director of Social Services

Solaris HealthCare Merritt Island is now hiring. We are looking for caring and compassionate individuals to provide outstanding care to our residents each and every day. The primary purpose of your job position is to plan, organize, develop, and direct the overall operation of our Solaris HealthCare’s Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.

Responsibilities

Plan, organize, develop, and direct the overall operation of the Social Services Department; Assure that medically related emotional and social needs of residents are met/maintained on an individual basis; Develop and implement policies and procedures for identification of medically related social and emotional needs; Participate in discharge planning, development and implementation of social care plans and resident assessments; Interview residents/families as necessary; Refer residents/families to appropriate social service agencies; Provide consultation to staff, community agencies, etc.; Coordinate social service activities with other departments; Maintain quality working relationship with medical profession and other health related facilities; Ensure compliance with federal, state, and local standards and regulations; Review and develop plan of correction for social service deficiencies; Assist in quality assurance and infection control programs

Requirements

Caring and compassionate; Qualified social worker with a minimum of a bachelor's degree in social work or a human services field (e.g., sociology, gerontology, special education, rehabilitation counseling, psychology)

Nice to have

Two (2) years' experience in a supervisory capacity in a long-term care or hospital setting
