Hospice RN - Evenings - PRN - Racine/Kenosha

Responsibilities

Develops and maintains a therapeutic relationship throughout the healthcare continuum; Facilitates the patient and family’s right to receive quality, cost effective care; Demonstrates a commitment to meeting the learning needs of patients and families; Uses nursing process as a problem-solving model; Completes assessment and identifies appropriate patient problem; Develops, implements, and evaluates plan of care; Prioritizes and organizes patient care; Safely implements interventions and procedures; Applies ethical decision making; Delegates correctly to unlicensed assistive personnel; Demonstrates respect and understanding for peers and other clinical disciplines; Contributes to effective operation of the unit

Requirements

Registered Nurse (RN); Current CPR; Valid driver’s license issued by the Division of Motor Vehicles; Graduate from an accredited school of nursing; Earn 5.0 continuing education credits annually; 1 year of medical nursing experience with various age groups; Good communication and interpersonal skills; Good documentation skills; Computer skills; Good time management and organizational skills; Familiarity with patient care equipment; Ability to work weekends, holidays, and different shifts; Ability to take on-call assignments; Ability to handle multiple tasks and complete work within short timeframes; Ability to respond quickly to patient needs; Ability to lift up to 100 pounds occasionally and/or in excess of 50 pounds frequently and/or in excess of 20 pounds constantly; Must be able to travel to all areas within the Advocate service area in all extremes of weather on all types of roads; Must be willing to work occasional overtime; Required to make visits to private residences; Must be able to assist with ambulation including turning and transferring patients up to 200 pounds; Ability to lift up to 35 pounds without assistance for direct patient care
