In-Home Health - Nurse Practitioner or Physician Assistant (Full Time) - Washington (Traveler)

Signify Health, part of CVS Health®, is seeking a Full-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. In this role, you’ll be primarily traveling - up to 100% of the time - across a defined coverage area, as defined by business needs. This may require overnight hotel stays, car rentals and/or flights, depending on the prescribed coverage area. Holding multiple state licenses will be required, as dictated by business needs.

Responsibilities

Provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care; Primarily travel up to 100% of the time across a defined coverage area, which may require overnight hotel stays, car rentals and/or flights; Conduct 45-minute visits including medication and medical history review, physical evaluation, and additional diagnostic tests (i.e., diabetic eye exams, spirometry, etc.) if ordered by the person's health plan; Use an iPad and an intuitive clinical workflow for each evaluation; Conduct virtual visits as needed; Provide all Signify Health services as indicated including Diagnostic & Preventive Services products (i.e., spirometry, DEE, etc.); Be flexible to travel locally and within licensed states, up to 100% of the time

Requirements

Active, unrestricted license(s) in coverage area(s); Board certification (required); Able to obtain and maintain multi-state licenses in coverage area, as required; Able to cover all counties within Washington and flex to other areas of business need; Able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
