In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Oswego/ Syracuse, NY

Signify Health, part of CVS Health®, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered, additional diagnostic tests. The clinician will use an iPad and an intuitive clinical workflow for each evaluation and is not required to prescribe medicine, order lab tests or alter treatment regimens.

Responsibilities

Provide In-Home Health Evaluations, including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered; Engage with people in the comfort of their own homes and help bridge gaps in care; Build trusted relationships to make people healthier; Conduct virtual visits as needed; Be flexible to travel locally and within licensed states, as needed; Provide Signify Health services including Diagnostic & Preventive Services products (i.e., spirometry, DEE, bone density screening, etc.)

Requirements

Active, unrestricted license(s) in coverage area(s); Board certification; Able to cover Oswego/Syracuse, NY and surrounding area; Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants
