In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) - Pueblo, CO

Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests.

Responsibilities

Bring your heart into every visit; Be part of efforts to visit millions of people nationwide; Work with Signify Health as a clinician colleague; Be flexible to travel locally and within licensed states; Conduct virtual visits as needed; Provide all Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)

Requirements

Active, unrestricted license(s) in coverage area(s); Board certification; Able to cover the following primary locations: Fremont, Saguache, Custer, Alamosa, Costilla, Huerfano, Pueblo, Crowley, Otero, Las Animas
