In-Home Nurse Practitioner or Physician Assistant Full Time - Erie

Signify Health, part of CVS Health®, is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

Responsibilities

Provide In-Home Health Evaluations; Engage with people in the comfort of their own homes and help bridge gaps in care; Build trusted relationships to make people healthier; Conduct medication and medical history review, physical evaluation, and additional diagnostic tests as ordered by the health plan; Use an iPad and intuitive clinical workflow for each evaluation; Travel locally and within licensed states, as needed; Conduct virtual visits as needed; Provide Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)

Requirements

Active, unrestricted license(s) in coverage area(s); Board certification; Able to cover the following primary locations; Comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants; Flexible to travel locally and within licensed states, as needed
