In-Home Nurse Practitioner or Physician Assistant Part Time - Huntington

Signify Health, part of CVS Health, is seeking a Part Time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations. The visit lasts about 45 minutes and includes medication and medical history review, a physical evaluation, and additional diagnostic tests if ordered by the person's health plan. The clinician uses an iPad and an intuitive clinical workflow, is not required to prescribe medicine or alter treatment regimens, and may need to travel locally and conduct virtual visits as needed.

Requirements

Active, unrestricted license(s) in coverage area(s); Board certification; Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
