In-Home Nurse Practitioner or Physician Assistant (Per Diem) - Huron County, MI

Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.

Responsibilities

Conduct in-home health evaluations (approximately 45 minutes); Review medication and medical history; Perform physical evaluation; Conduct additional diagnostic tests if ordered by the person's health plan; Conduct virtual visits as needed; Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products; Travel locally and within licensed states as needed; Build trusted relationships with health plan members

Requirements

Active, unrestricted license(s) in coverage area(s); Board certification (required); Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
