In-Home Physician Part Time - Allentown - Region

Signify Health, part of CVS Health, is seeking a Part Time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. The 45-minute visit includes a medication and medical history review, a physical evaluation, and, if ordered by the person's health plan, additional diagnostic tests. Candidates must be able to comfortably and safely perform duties in home environments that may contain pets, secondhand smoke and other potential airborne allergens or irritants.

Responsibilities

Conduct 45-minute in-home health evaluations, including medication and medical history review, physical evaluation, and additional diagnostic tests if ordered; Use an iPad and intuitive clinical workflow for each evaluation; Connect one-on-one with members and answer health questions; Build trusted relationships to improve health outcomes; Be flexible to travel locally and within licensed states as needed; Conduct virtual visits as needed; Provide Signify Health services including Diagnostic & Preventive Services products such as spirometry, DEE, and bone density screening

Requirements

MD or DO; Active, unrestricted license(s) in coverage area(s); Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
