In-home Traveler Physician

Signify Health, part of CVS Health, is seeking a Full-time Physician to provide In-Home Health Evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care. In this role, you'll be primarily traveling - up to 100% of the time - across a defined coverage area. The visit lasts about 45 minutes and includes a medication and medical history review, a physical evaluation, and additional diagnostic tests if ordered. Responsibilities

Perform In-Home Health Evaluations; Conduct medication and medical history review; Perform physical evaluations; Conduct additional diagnostic tests (diabetic eye exams, spirometry) as ordered by health plan; Use iPad and intuitive clinical workflow; Conduct virtual visits as needed; Provide all Signify Health services including Diagnostic & Preventive Services products; Travel up to 100% locally and within licensed states; Build trusted relationships with health plan members

Requirements

MD or DO; Active unrestricted license(s) in coverage area(s); Able to obtain and maintain multi-state licenses in coverage area; Able to cover all counties within New Hampshire and flex to other areas of business need; Primary residence in state of New Hampshire; Comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants; Flexible to travel locally and within licensed states, up to 100% of the time
