Pharmacist 2, Clinical Spec - Pediatric Cardiac ICU

Provisions comprehensive clinical pharmacy services related to their area of practice to optimize safe, effective and cost-effective drug therapy. Participates as a member of an inter-disciplinary patient care team, provision of pharmacotherapeutic and pharmacokinetic consults, patient and provider education, and the development of best practice guidelines and policy related to pharmacotherapy. Serves as a leader for the department in their specialty area of practice, organizing appropriate departmental initiatives and providing education and teaching for pharmacy colleagues, providers and trainees at all levels of practice. Contributes to the overall objectives of the department as an integrated member of the pharmacy practice team.

Responsibilities

Provides comprehensive clinical pharmacy services for a specialized area of pharmacy practice; Participates on inter-disciplinary patient care team rounds; Provides consultative input on safe and effective drug therapy; Monitors assigned patient populations for clinical progress; Provides patient education and discharge counseling; Authorizes and verifies orders, and supervises drug distribution when appropriate; Documents clinical pharmacy activities in the medical record or other systems; Provides clinical leadership in specialty area by serving as mentor, coach and role model; Directs and supervises the work of technicians; Conducts QA, MUE, or other research to document the value and contribution of clinical pharmacy services; Serves as preceptor for pharmacy residency training programs; Provides didactic lectures to various divisions and departments; Resolves problems with appropriate and direct communication; Holds active membership in local, regional and national professional organizations; Serves on departmental, hospital or medical center committees or task forces

Requirements

Doctor of Pharmacy degree required; Or equivalent combination of education and experience; New York State Pharmacy License upon hire required; Board certification in the most relevant specialty area within 2 years required

Nice to have

Specialty residence training preferred; New resident graduates or clinical specialists in their first 3-5 years of practice preferred
