Physical Therapist (PT) - Home Health

Part-time, PRN physical therapist position with Advocate Health At Home, Cleveland region home care physical therapy, located in Shelby, NC. The role involves managing an assigned patient caseload, evaluating patients, developing and implementing plans of care, and coordinating a multidisciplinary clinical team in patients' homes.

Responsibilities

Responsible for the daily management of assigned patient caseload, ensuring timely care and documentation per the plan of care and MD orders; Collects data from patient assessments in the home setting, telephonic contacts, and/or related EMR information; Analyzes data to identify issues and resources, and identifies high-risk patients requiring additional intervention and case conferencing; Prioritizes multi-issue situations based on patient health, patient/staff safety, and service timeliness; Evaluates the PT patient to determine rehabilitation needs and potential, and develops a plan of care with the patient's physician; Establishes and implements the plan of care, performs certified PT services, and directs care coordination of the multidisciplinary team, including supervision of LPTAs and/or HHAs; Maintains communication with the multidisciplinary team regarding schedule changes, patient data, and physician orders; Incorporates short- and long-term goals into therapy, documented in visit notes, monthly progress notes, and discharge summaries; Supports communication and collaboration with physician oversight and branch supervision; Performs ordered treatments, therapies, and procedures in accordance with State Physical Therapy Practice Act, Home Health Conditions of Participation, JCAHO, Medicare Guidelines, OASIS, and agency policies

Requirements

Doctorate degree in Physical Therapy from an accredited school or education requirements commensurate with the date of graduation; Maintains continuing education requirements as mandated by applicable state professional standards; Physical Therapy License in the State of applicable state or South Carolina, must be licensed in the state where you work; Basic Life Support for Healthcare Provider certification from the American Heart Association; Current state driver's license and auto insurance at all times; Demonstrates knowledge of principles of growth and development and the skills and competency appropriate to the ages, culture, developmental stages, and special needs of the patient; Ability to walk, stand, sit, lift, reach, stoop, bend, push, and pull; Must be able to lift and support 50 pounds in handling patients, medical equipment, and supplies; Ability to travel extensively during all seasons to assigned patients' residences; Must speak English in good understandable terms; Intact sense of sight, hearing, smell, and touch; Finger dexterity; Critical thinking and ability to concentrate; Must be able to respond quickly to changes in patient and/or unit conditions; Ability to work a flexible schedule to meet patient needs

Nice to have

1-year experience as a licensed PT in a clinical care setting or home health; 1 year of pediatric experience within the past 5 years, or education and training may be provided by Advocate Health At Home
