Physical Therapy Assistant (PTA)

Adaptive is building an AI-native home healthcare platform that enables clinicians to focus on delivering best-in-class patient care by leveraging software and innovative support solutions. The company is building its team in Detroit. The role is to deliver excellent home health care to patients in Detroit and set the clinical bar, documentation standard, and referral-source trust for the market. The platform offers customized schedules, optimized routing, ambient AI scribing, and after-visit voice documentation to reduce charting time.

Responsibilities

Deliver skilled PTA visits in the home under the supervising PT's plan of care; Execute treatment plans with a focus on functional mobility and measurable progress: gait/balance, transfers, strengthening, functional mobility, home safety, fall prevention; Set/track objective progress within PTA scope and communicate updates/concerns to the supervising PT; Drive multidisciplinary coordination: escalate changes in condition, align on orders, and keep the care team moving; Coach patients and caregivers with practical home programs; Document promptly and thoroughly; Carry a full, high-quality caseload, ramping to ~30 productivity points/week at target visit quality; Participate in case conferences/team huddles to improve quality; Build referral-source relationships with discharge planners, ALFs, and physicians

Requirements

Active Michigan PTA license (in good standing); Reliable transportation, valid driver's license, auto insurance; Able to carry/transport basic therapy equipment as needed; Competency in core home health PTA scope; Safety-first mobility work: transfers, gait training, balance/fall prevention, assistive device recommendations; Strengthening, functional mobility, and home safety assessment/modification education; Patient/caregiver coaching with practical home programs

Nice to have

Home health experience (strongly preferred); High ownership; Open mindset and willingness to shape playbook and refine SOPs; Tech-forward, willing to leverage software to accelerate charting; Trust-builder with patients, families, and referral partners
