Responsibilities

Provide comprehensive care coordination for hospitalized patients.; Conduct high-risk screenings and assessments to identify patient needs.; Arrange and document post-acute care services , ensuring smooth transitions of care.; Collaborate with a multidisciplinary healthcare team to support optimal patient outcomes.

Requirements

1 year case management experience; Associate of Science in Nursing required; Bachelor of Science in Nursing preferred; Basic Life Support (BLS) required; FL Licensed Registered Nurse required
