Social Worker - PRN - Days

The Social Worker will perform a variety of duties to address the social, psychological and economic needs of children and their families, in both an inpatient and outpatient setting. The person in this role is concerned with continuity of care which usually involves discharge planning for children who are inpatients and their families to ensure they understand and follow the recommended treatment plan, and referrals to community agencies and programs.

Responsibilities

Perform a variety of duties to address the social, psychological and economic needs of children and their families, in both an inpatient and outpatient setting; Provide coverage for teammates who are out of the office; Work in a variety of areas within the hospital; Assist families in meeting their needs through short term, crisis oriented counseling, referrals to community agencies, programs and services, and educating patients/families about illness, hospital procedures and community resources; Help plan for future health care needs of the child whether or not admitted to the hospital; Consult with other professional staff to assure that they understand the strengths and needs of patients and families; Assess the medical care/history of the child and family to assist medical staff with diagnosis and treatment plan

Requirements

1+ years of previous social work or related field experience required; Master's Degree MSW required; One of the following: License Social Worker Advance Practice - APSW - (121) - State of Wisconsin; License Social Worker Clinical (123) - State of Wisconsin; License Social Worker Independent (122) - State of Wisconsin
