Virtual Clinician Home Evaluations - Nurse Practitioner or Physician Assistant - WA

Signify Health, part of CVS Health®, is hiring full-time, board-certified nurse practitioners to conduct Video Visits for Medicare Advantage, Medicaid, and certain commercial plan members. During our Video Visits, you’ll conduct an approximately 30-minute visit, including a medication and medical history review, allowing time to educate members about their health and answer their questions. In this role, you will: Conduct online Video Visits with health plan members, working in a private space with adequate internet connection; Provide 40 hours of availability weekly, consistently providing 320+ capacity units per month; Execute a strong visit completion rate, consistently pacing 220+ completed visits per month; Maintain training, affiliation, and compliance with Video Visit requirements.

Requirements

Board-certified nurse practitioner; Conduct approximately 30-minute Video Visits including medication and medical history review; Provide 40 hours of availability weekly, consistently providing 320+ capacity units per month; Execute a strong visit completion rate, consistently pacing 220+ completed visits per month; Maintain training, affiliation, and compliance with Video Visit requirements; Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s); Consistently engage with regular manager, team and clinical meetings; Be accountable to Signify Health operational, clinical, and member experience key performance indicators and metrics; Maintain active state licenses in coverage areas; Maintain board certification; Primary coverage areas include but are not limited to Washington; Able to obtain and maintain multi-state licenses in coverage area, as required; Dedicated to providing Video Visits (telehealth) services only; Meet minimum requirements for remote visit delivery including broadband connectivity with video and audio enabled, a quiet setting with a neutral background, and ability to uphold health plan member privacy per CVS Health guidelines; Ability to interact remotely, sit, write, operate computer and phone, speak intelligibly, and hear member responses; Specific vision abilities include ability to view and read a computer screen, iPad, and other electronic devices; Effective verbal, written, and electronic communication skills; Ability to work without direct supervision and practice autonomously; Proficiency with information management and technology

Nice to have

FNP, AGNP certification preferred for NPs; Experience with Signify Health In-Home Health Evaluations, Video Visits, in-home visits, telehealth visit delivery, or a similar format (preferred)
